Boston, MA: Houghton Mifflin Company, 1919. as is (printing defect). 283, illus., index, ink notation inside front board, rear board weak, boards worn, small tears at top & bottom spine edges. There is a printing defect at the end of the volume; where pages 256, 257, 260, 261, 264, 265, 268, and 269 should appear, there are duplicate copies of pages 214, 211, 210, 207, 222, 219, 218, and 215. Dr. White was Chair of Clinical Surgery at the University of Pennsylvania. He and Theodore Roosevelt were close friends, having bonded through a love of college athletics. "In trust he [White] could no more tolerate a jest at the expense of Roosevelt than he could tolerate a jest at the expense of his profession. "
